Scheduled Weekly Hours: 36 Hospice Admission Nurse (RN) Location: La Crosse, WI FTE: 0.9 (72 hours per two weeks) Shift: 9-hour shifts (4/week) Holiday: Rotating every 3rd holiday Join Our Dedicated Hospice Team At Emplify Health by Gundersen, hospice care is about compassion, dignity, and support for patients and their loved ones during life’s most meaningful moments. We are seeking a Hospice Admission Nurse (RN) to welcome patients and families into our hospice program and ensure a smooth, supportive transition to care. Hospice services are available within a 50-mile radius of La Crosse and as an Admission Nurse, you will be the first point of contact, guiding patients and caregivers through the admission process with empathy and expertise. Comprehensive Support: Help patients and families understand hospice services and what to expect. Collaborative Team: Work closely with a multidisciplinary team to ensure seamless transitions. What You Will Do Complete hospice admissions, assist with patient visits, and provide education/program information for potential Hospice patients Provide education and support to patients and caregivers during end-of-life transitions Collaborate with a multidisciplinary team including nurses, social workers, chaplains, bereavement coordinators, and hospice aides Work primarily daytime hours and rotating holidays What You Need Associate Degree in Nursing Minimum of 1 year RN experience RN licensure to practice in Wisconsin upon hire RN licensure to practice in Minnesota within 3 months of hire What You Will Get Gundersen’s generous compensation and benefits package, including a top-rated retirement plan Growth opportunities and access to Gundersen’s Career Development Center A rewarding role where you make a difference from the very first interaction Ready to join us? Requirements

  • Associate Degree in Nursing
  • Minimum of 1 year RN experience
  • RN licensure to practice in Wisconsin upon hire
  • RN licensure to practice in Minnesota within 3 months of hire
