About The Position

We are seeking a dedicated and compassionate Mental Health Homeless Outreach & SOAR Case Manager to support individuals experiencing chronic homelessness and mental illness. This position focuses on outreach, case management, and assisting individuals with accessing mental health services, housing resources, and SSI/SSDI benefits through the SOAR (SSI/SSDI Outreach, Access, and Recovery) program. This role works closely with multidisciplinary teams and community partners to promote recovery, stability, and self-sufficiency.

Requirements

  • High school diploma and at least 12 college credits in a social science field (such as psychology, sociology, social work, criminal justice, or related field) and two (2) years of experience working with individuals with mental illness,  OR High school diploma and five (5) years of mental health direct care experience in human services (case management experience preferred).
  • Access to reliable transportation.
  • Ability to obtain PA Act 33 and Act 34 clearances.
  • Strong organizational, communication, and advocacy skills.
  • Ability to work independently and collaboratively.

Nice To Haves

  • Bachelor’s degree in social work, psychology, sociology, criminal justice, or related field.
  • Experience working with individuals experiencing homelessness.
  • Experience with SSI/SSDI or SOAR applications.

Responsibilities

  • Provide case management services to individuals experiencing homelessness and mental illness.
  • Develop individualized service plans based on each person’s needs, goals, and preferences.
  • Connect individuals with mental health treatment, housing, medical care, and community resources.
  • Accompany individuals to appointments to ensure engagement with services.
  • Advocate for access to benefits, services, and supports.
  • Maintain accurate and timely case documentation, reports, and service records.
  • Respond to urgent or emergency situations and make appropriate referrals.
  • Coordinate care with community agencies and service providers.
  • Participate in meetings, committees, and collaborative initiatives related to homeless services.
  • Identify eligible individuals and assist with SSI/SSDI applications.
  • Establish protective filing dates and gather required documentation.
  • Obtain releases and coordinate with medical and treatment providers.
  • Prepare and submit Medical Summary Reports and complete application packets.
  • Schedule and support individuals through SSA appointments.
  • Maintain communication with Social Security and disability determination representatives.
  • Assist approved applicants with accessing housing, treatment, and ongoing supports.
