Qualifications An Average Day: (Includes, but not limited to) Provide homemaking activities that allow patients to remain safely in their homes. Keep the home clean and organized through light housekeeping (vacuuming, sweeping, mopping, dusting). Prepare and serve meals according to patient/family needs and the plan of care. Wash dishes, clean the kitchen after meals, and care for laundry and linens. Offer companionship, conversation, and emotional support, including reading or listening. Observe and report changes in a patient’s condition to the RN Case Manager. Support safety, hygiene, and good nutrition practices within the home. Help with organization of the home and provide simple plant care as needed. Complete visit documentation in accordance with hospice guidelines. Other duties as assigned
