A Senior Home Health Physical Therapist will follow appropriate practices, to perform evaluations, timely re-evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. The therapist is required to complete supervisory visits for PT/PTA personnel, participate in interdisciplinary case management serving as an example of excellence in case management, complete chart reviews, participate in meetings as required by facility/department, engage in other initiatives to improve quality outcomes, and take part in recruitment and new hire activities as needed. Advanced competency must also be demonstrated with OASIS as well as clinical supervision.
