A Senior Home Health Physical Therapist will follow appropriate practices, to perform evaluations, timely re-evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. The therapist is required to complete supervisory visits for PT/PTA personnel, participate in interdisciplinary case management serving as an example of excellence in case management, complete chart reviews, participate in meetings as required by facility/department, engage in other initiatives to improve quality outcomes, and take part in recruitment and new hire activities as needed. Advanced competency must also be demonstrated with OASIS as well as clinical supervision. What you will do Performs evaluations, determines PT diagnoses, develops plan of care, and performs physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.) Executes and modifies care plan appropriately to discharge patients at optimal level of care. Provides reassessments including OASIS assessments at appropriate time points to determine the level of functioning. Updates and revises the plan of care in consultation with the physician and interdisciplinary team members. Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings, as assigned. Supervises Physical Therapy Assistants and Home Health Aides in providing services including completion of required supervisory visit documentation. Participates in training and support of all team members, volunteers and students. Provides education to internal therapy team; actively engages in external recruitment opportunities; participates in RHS/community events to increase awareness of home health. Actively participates in new hire orientation process including serving as Preceptor for PT/PTA. Serves as clinical instructor annually.
