About The Position

BJC Home Care is seeking a home health registered nurse to provide 1:1 patient care to an under-served community, offering work-life balance and flexible scheduling. This role allows nurses to build meaningful relationships with patients in a supportive team environment. BJC Home Care provides a complete range of home care services, including skilled nursing, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment, and high-tech respiratory care. It serves over 25 counties in Missouri and Illinois, making it the largest home care network in the region. The Metro St. Louis Intermittent Home Care Department offers home visits with 24-hour on-call nursing supervision, utilizing a JCAHO accredited, multi-disciplinary approach combining technology with the benefits of home care.

Requirements

  • Nursing Diploma/Associate's in Nursing
  • Less than 2 years of experience
  • No Supervisor Experience
  • Valid Driver's License
  • RN (Registered Nurse)
  • Willingness to provide coverage in all St. Louis regions during on-call
  • Registration with the Family Care Safety Registry
  • Ability to work in a constant state of alertness and in a safe manner

Responsibilities

  • Evaluate the client and furnish services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques.
  • When assigned as case manager, coordinate all aspects of care related to that patient.
  • Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
  • Develop, implement, and document individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care.
  • Proactively plan and ensure communication of the plan of care across the continuum of care.
  • Promote respect, equity and empathy in interactions with diverse and vulnerable populations through care delivery (e.g. support for emotional, spiritual, and cultural preferences of patient, family and/or caregivers).
  • Practice collaborative problem solving, service recovery and advocacy for patient family centered continuity of care.
  • Implement care by integrating data from the interprofessional team and critical thinking in a safe and timely manner.
  • Evaluate changes in patient's condition, inform and collaborate with family and/or caregivers, and communicate with interprofessional team as changes occur in plan of care.
  • Update plan of care in EMR.
  • Evaluate current nursing care to ensure evidence-based practice and quality patient outcomes.
