Requirements

  • Current RN license in good standing in the state of practice.
  • Associate's degree in Nursing or related field.
  • 2+ years of experience as a Home Health Administrator, Director of Operations, or equivalent leadership role.
  • Strong knowledge of CMS regulations and home health compliance requirements.
  • Proven leadership, communication, and problem-solving skills.

Nice To Haves

  • Experience as an interim or turnaround leader in home health.
  • Bachelor's degree in Nursing or related field.
  • Background in managing multi-site or high-volume agencies.
  • Track record of improving operational metrics in a short time frame.

Responsibilities

  • Direct the day-to-day operations of the home health branch, ensuring compliance with all applicable federal, state, and accreditation standards (CMS, Medicare, Joint Commission, etc.)
  • Oversee the coordination of patient admissions and discharge planning to ensure a seamless patient experience
  • Provide clinical oversight and leadership to interdisciplinary home health staff, including RN Case Managers, LPNs, therapists (PT, OT, ST), aides, and administrative personnel
  • Interview, hire, train, supervise, and evaluate staff while fostering a supportive and collaborative team culture
  • Promote best practices in skilled nursing, chronic disease management, and patient-centered home care
  • Monitor and improve operational efficiency, service delivery, and patient satisfaction
  • Lead performance management efforts, including staff evaluations, scheduling, workload distribution, and addressing employee concerns
  • Ensure continuous survey readiness and participate in quality assurance and performance improvement (QAPI) initiatives
  • Manage branch financial performance, including budgeting, payroll, supply costs, and referral development
  • Build and maintain strong relationships with physicians, hospital discharge planners, and community referral sources
