About The Position

A Home Health Registered Nurse (RN) will deliver comprehensive, patient-centered care in a home setting, managing patients’ medical needs and promoting overall health and well-being. This role requires strong clinical skills, excellent communication abilities, and the capability to work independently while collaborating with an interdisciplinary healthcare team.

Requirements

  • Graduate of an approved school of professional nursing who is licensed as a Registered Nurse by the state in which practicing.
  • Minimum of two (2) years nursing experience in Home Health required.
  • Ability to supervise and direct members of client care team.
  • Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
  • Ability to deal tactfully with clients and the community.
  • Demonstrate organizational and time management skills.
  • Licensed driver with automobile insurance.

Responsibilities

  • Administer medications and treatments as prescribed by physicians and in accordance with home health care protocols.
  • Perform routine and specialized nursing tasks, including wound care, catheter insertion, and monitoring vital signs.
  • Assist in developing and implementing individualized care plans in collaboration with the patient, family, and healthcare team.
  • Monitor and document patient progress, response to treatments, and any changes in condition, reporting significant findings.
  • Provide education and support to patients and their families on disease management, medication use, and home care techniques.
  • Assist with patient mobility, transfers, and activities of daily living as needed, ensuring safety and comfort.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
  • Collaborate with LPNs, physicians, and other healthcare professionals to ensure coordinated and comprehensive care.
  • Respond to patient needs and changes in condition promptly, providing appropriate interventions and support.
  • Adhere to infection control and safety procedures to protect patients and staff.
  • Perform all other duties assigned.
