About The Position

Alpha Home Health & Hospice is seeking a full-time Home Health Registered Nurse (RN) for their King County area Home Health division. This role focuses on providing one-on-one patient care for complex medical needs directly in patients' homes. The position offers autonomy in managing one's own schedule within a supportive leadership and inclusive culture. Requirements

  • Washington RN licensure
  • Current BLS certification
  • Valid driver’s license and insured vehicle

Nice To Haves

  • Home Health/Hospice agency experience preferred, but not required

Responsibilities

  • Conduct initial and ongoing patient assessments in the home setting
  • Develop and update individualized care plans in collaboration with the care team
  • Administer medications, treatments, and injections as prescribed
  • Perform wound care and dressing changes and other complex care
  • Monitor vital signs and symptoms, reporting changes to the physician
  • Educate patients and caregivers on disease processes, medications, and self-care
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Document all visits and care provided in the EMR
  • Ensure compliance with agency policies, Medicare/Medicaid, and other regulations
  • Participate in case conferences and interdisciplinary team meetings
  • Provide emotional support and promote patient independence
