About The Position
PRN Sterling House of Norman - 1701 East Alameda St. Norman, OK 73120 http://www.brookdaleliving.com/sterling-house-oklahoma-city-north.aspx Opportunity will also cover Norman, Shawnee and the Midwest City area Key responsibilities include: Evaluates the patient & home health environment to determine required services Serves as the patient’s advocate by providing information Responsible for timely completion & synchronization of OASIS visits as required & developing the POC based on findings from evaluation visit Involves patient & patient’s family in developing the plan of care
Requirements
- RN state licensure in good standing
- Bachelor or Masters level degree in Psych & experience in mental health
- Medicare, home healthcare, and OASIS experience
- 1 year of Psych RN experience within the last 2 years
Responsibilities
- Evaluates the patient & home health environment to determine required services
- Serves as the patient’s advocate by providing information
- Responsible for timely completion & synchronization of OASIS visits as required & developing the POC based on findings from evaluation visit
- Involves patient & patient’s family in developing the plan of care
