What Makes This PRN Role Different Build your own schedule—work when it works for you Focused, one-on-one patient visits Strong clinical guidance and approachable leadership Competitive hourly pay Free continuing education and skill-building opportunities Pathways for future growth within the organization How You’ll Make an Impact Working under the direction of an RN or physician, you’ll provide skilled home health services by: Delivering care based on individualized plans and physician orders Performing skilled nursing tasks aligned with LPN scope of practice Observing, assessing, and reporting changes in patient condition Educating patients and caregivers on medications, treatments, and self-care Promoting patient safety and identifying potential risks in the home Documenting visits thoroughly and accurately in the clinical system Partnering with RN Case Managers for care coordination and guidance Supporting interventions aimed at reducing unnecessary hospital visits What You Bring to the Role Graduation from an accredited LPN nursing program Active, unrestricted LPN license At least one year of hands-on nursing experience Home Health or Hospice background preferred, not required Willingness to travel throughout the service area (approximately 50%) Reliable transportation, valid driver’s license, and current auto insurance Ability to meet the physical demands of home-based nursing Why Your Work Here Matters For many patients, home is where healing happens. Requirements

  • Graduation from an accredited LPN nursing program
  • Active, unrestricted LPN license
  • At least one year of hands-on nursing experience
  • Willingness to travel throughout the service area (approximately 50%)
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to meet the physical demands of home-based nursing
