Requirements
- Graduation from an accredited LPN nursing program
- Active, unrestricted LPN license
- At least one year of hands-on nursing experience
- Willingness to travel throughout the service area (approximately 50%)
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability to meet the physical demands of home-based nursing
Nice To Haves
- Home Health or Hospice background preferred, not required
Responsibilities
- Delivering care based on individualized plans and physician orders
- Performing skilled nursing tasks aligned with LPN scope of practice
- Observing, assessing, and reporting changes in patient condition
- Educating patients and caregivers on medications, treatments, and self-care
- Promoting patient safety and identifying potential risks in the home
- Documenting visits thoroughly and accurately in the clinical system
- Partnering with RN Case Managers for care coordination and guidance
- Supporting interventions aimed at reducing unnecessary hospital visits
