Requirements
- Active Minnesota Physical Therapist license.
- Valid driver's license and reliable transportation.
- Passion for individualized, patient-centered therapy.
Nice To Haves
- Home health experience preferred, but outpatient and inpatient PTs strongly encouraged to apply.
Responsibilities
- Perform in-home mobility and functional assessments.
- Develop and execute individualized rehabilitation plans.
- Provide hands-on therapy, strengthening exercises, and mobility training.
- Recommend home safety modifications and adaptive equipment.
- Communicate with nurses, caregivers, and the care team to ensure coordinated care.
