Are you looking for a change from clinical 9-5? Signature Healthcare at Home has an exciting opportunity to add a Physical Therapist to our team! This position covers Whatcom County (Bellingham, Lynden, Ferndale, Blaine, Birch Bay, Everson, Sudden Valley) as the coverage area. The ideal candidate is someone with home health experience and enjoys work with a high-level of autonomy. Signature Healthcare at Home is a leading provider of home health in Oregon and Washington. Duties and Responsibilities Assumes the responsibility for the on-going interdisciplinary and individualized plan of care in partnership with the patient, family/representative and/or caregiver(s). Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment. Provides patient, family, and/or caregiver counseling, education, and progress summary based on the attainment of goals. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences. Supervises physical therapy assistant(s) according to organizational policy and in alignment with state regulations. Completes daily documentation, scheduling Knowledge, Skills and Experience Possess and maintains licensure to practice Physical Therapy in the state of Washington. Graduated from an accredited CAPTE or successor organization with at least a baccalaureate degree for legacy programs or a doctorate's degree in physical therapy. Possesses and maintains a valid and active BLS CPR certification from the American Heart Association or American Red Cross. Possesses and maintains a valid drivers' license and personal automobile with insurance for daily travel as a requirement. Schedule, Pay and Benefits Part-time or PRN flexible scheduling. Requirements

  • Possess and maintains licensure to practice Physical Therapy in the state of Washington.
  • Graduated from an accredited CAPTE or successor organization with at least a baccalaureate degree for legacy programs or a doctorate's degree in physical therapy.
  • Possesses and maintains a valid and active BLS CPR certification from the American Heart Association or American Red Cross.
