About The Position

The Home Health Physical Therapist plays a critical role in delivering personalized rehabilitation services to patients within the comfort of their own homes across the United States. This position focuses on assessing, planning, and implementing therapeutic interventions that promote mobility, reduce pain, and improve the overall quality of life for individuals recovering from injury, surgery, or managing chronic conditions. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care tailored to each patient's unique needs and goals. By providing expert guidance and support, the therapist empowers patients to regain independence and achieve optimal functional outcomes. This role requires a compassionate, patient-centered approach combined with clinical expertise to effectively manage diverse caseloads in varied home environments

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance

Nice To Haves

• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.
