About The Position

We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position serving adult and geriatric patients. This role offers independence, strong support, and good pay.

Requirements

• Bachelor’s, Master’s, or Doctorate in Physical Therapy
• Active PT license in the state
• Minimum 2 years of PT treating experience
• Must be a U.S. Citizen or Green Card holder (no exceptions)
• Comfortable with driving between patient homes (Most patients are close together; occasional 20–40 min drive. Mileage reimbursed.)

Responsibilities

• Evaluate patients referred by physicians using diagnosis-specific assessments
• Provide in-home PT visits for adult and geriatric patients
• Complete timely documentation (evaluations, progress notes, time & mileage)
• Schedule and manage your daily patient visits
• Collaborate with physicians and interdisciplinary team members
• Communicate patient progress and updates as needed
