We are strengthening our Santa Clara County team and are seeking an Occupational Therapist who wants flexibility, independence, modern tools, and an organized home health structure behind them. Requirements
- Current California Occupational Therapist license.
- Valid California driver’s license.
- Strong clinical judgment, organization, and time-management skills.
- Home health experience is preferred, but strong outpatient, inpatient, or rehab OTs are encouraged to apply.
Responsibilities
- Independently perform occupational therapy evaluations and reassessments.
- Develop and manage individualized plans of care.
- Deliver skilled therapy focused on ADLs, safety, fall prevention, and independence.
- Address home safety, adaptive equipment, and functional training.
- Communicate directly with physicians and the interdisciplinary team.
- Educate patients and caregivers.
- Complete documentation in Kinnser supported by AI tools.
- Serve as the primary OT voice in patient care planning.
