Provide skilled nursing care to patients in their homes Collaborate with a multi-disciplinary team of therapists, CNAs, and providers Complete accurate and timely documentation Educate patients and families on disease processes and care plans Deliver high-quality, patient-centered care with a focus on independence and safety What should I have to be considered?
