We are seeking an experienced Home Health Marketing Liaison with a strong background in the Los Angeles healthcare market and proven success in cardiology-related referral development. Requirements

  • 3–5 years of home health or healthcare marketing experience in the Los Angeles market (required)
  • Proven success working with cardiology practices or cardiac-related services (strongly preferred)
  • Established network of referral sources in LA and surrounding areas
  • Excellent communication, presentation, and negotiation skills
  • Self-motivated, organized, and results-driven
  • Compassionate, patient-first mindset
  • Valid driver’s license and reliable transportation

  • Bachelor’s degree preferred (Healthcare or Business-related field a plus)

Responsibilities

  • Develop and maintain referral relationships with cardiologists, hospitals, case managers, discharge planners, and physician offices across the LA market
  • Promote home health care services, emphasizing cardiac care and chronic disease management programs
  • Consistently achieve monthly and quarterly admissions and referral targets
  • Educate referral sources on clinical outcomes, patient benefits, and regulatory compliance
  • Monitor market trends, competitor activity, and payer mix
  • Collaborate with clinical and operations teams for seamless patient transitions
  • Document all sales activities and referral data in CRM systems
