About The Position

Join our dedicated team as an Intake Specialist and play a vital role in connecting patients to the care they need in the comfort of their homes. The Intake Specialist plays a critical role in the onboarding of new patients into home health services. This position ensures timely, accurate, and compassionate coordination of referrals, patient information, and communication between referral sources, patients, clinicians, and administrative staff.

Requirements

  • High school diploma or equivalent required; associate or bachelor's degree preferred.
  • Minimum of 2 years of experience in healthcare intake, preferably in home health or hospice.
  • Knowledge of medical terminology, insurance verification, and EMR systems.
  • Strong organizational skills and attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to multitask in a fast-paced environment.

Nice To Haves

  • Experience with home health EMR platforms
  • Familiarity with Medicare and regulations.

Responsibilities

  • Receive and process incoming referrals from hospitals, physicians, and other healthcare providers.
  • Verify insurance eligibility and benefits.
  • Ensure all required documentation is complete and compliant with regulatory standards.
  • Contact patients and/or caregivers to confirm services and gather necessary information.
  • Schedule initial assessments and coordinate with clinical staff.
  • Maintain accurate and up-to-date patient records in the EMR system.
  • Serve as a liaison between referral sources, patients, and internal departments.
  • Communicate effectively with clinical and administrative teams to ensure smooth transitions of care.
  • Provide updates to referral sources as needed.
  • Ensure all intake documentation meets Medicare and other payer requirements.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Assist with audits and quality assurance initiatives as needed.
