About The Position

For over 20 years, Home Care Providers has delivered exceptional in-home care focused on patient dignity, independence, and innovation. We are growing and seeking a detail-oriented Intake Coordinator to join our team. The Intake Coordinator is responsible for processing referrals, verifying insurance, and coordinating admissions in a fast-paced home health environment. This role works closely with scheduling staff and the RN Admission Manager to ensure timely and accurate patient onboarding.

Requirements

  • Medicare insurance knowledge required
  • 1–5 years of home health intake/referral experience required
  • Insurance verification experience required
  • Strong computer and EMR proficiency
  • Ability to work 11:00am–8:00pm schedule
  • Active driver’s license and reliable transportation
  • Highly organized with strong attention to detail

Responsibilities

  • Process and enter all incoming referrals accurately and efficiently
  • Verify insurance coverage and document confirmations
  • Triage service orders and Start of Care documentation
  • Obtain signed physician orders and required clinical documents
  • Research and secure missing referral information
  • Maintain accurate electronic records and referral logs
  • Perform additional duties as assigned
