Requirements

  • Education: High school diploma required; associate’s or bachelor’s preferred.
  • Experience: Minimum 2 years in healthcare — Home Health/Hospice intake experience required.
  • Skills & Knowledge: Proficient in EMR systems, medical terminology, and insurance procedures.
  • Detail-Oriented: Catch what others miss and keep processes running smoothly.
  • Strong Communicator: Confident, compassionate, and effective with patients, families, and providers.
  • Adaptable: Comfortable managing shifting priorities and occasional evening/weekend needs.

Responsibilities

  • Lead Patient Intakes: Collect and organize referral and medical documentation with precision.
  • Coordinate With Providers: Work closely with doctors, nurses, and care teams to ensure seamless care.
  • Handle Insurance & Authorizations: Verify coverage and secure approvals promptly to avoid delays.
  • Maintain Accurate Records: Enter and manage patient information in EMRs and digital systems.
  • Stay Compliant: Uphold HIPAA and privacy standards at all times.
  • Support Patients & Families: Communicate clearly, professionally, and compassionately.
