About The Position

The Home Health Coordinator – Internal provides admission support for all patients referred for home health services at Firelands Regional Medical Center. This individual has extensive knowledge of homebound admission criteria and requirements of the regulatory bodies. This individual serves as a resource to the health care team on home health issues. Effectively interacts with patients, families, case managers and physicians in communicating services provided to meet the individual client’s needs in a timely professional manner.

Requirements

  • Candidate must demonstrate comprehensive clinical understanding of home health services.
  • Registered Nurse licensed in the State of Ohio, preferred.
  • Other candidates must have previous healthcare experience.
  • Basic computer skills necessary.
  • Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy.
  • Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Demonstrated ability to work with all individuals on health care team.
  • Capable of working independently as well as part of the team.
  • Capable of adapting to changing environment.
  • Punctual attendance at assigned work location is required.
  • Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of FRMC emergency codes.
  • Ability to understand and effectively communicate in both written and oral form and handle sensitive telephone and face-to-face inquires.

Responsibilities

  • Utilizes good clinical assessment skills to identify and facilitate patient referrals to home health services. Appropriate patients will include, but are not limited to, those needing: Wound care, including V.A.C. Therapy
  • Diabetes education and related medication instruction
  • Diet therapy instruction
  • Assistance and care following surgery
  • Prosthetic and orthopedic rehabilitation
  • Assistance with speech and swallowing disorders
  • Care for heart problems and cardiac disease
  • Assistance and instruction in activities of daily living (ADL), including bathing, feeding and grooming
  • Medication management instruction
  • Stroke therapy (CVA)
  • Respiratory/Pulmonary education and related medication instruction
  • Hyperemesis
  • Ostomy care
  • Educates physicians, nurses, therapists as well as discharge planning professionals involved in the care of those patients listed above, as to the appropriateness of home health intervention.
  • Presents options to discharging patients that have been referred for home health services as to the available choices of home health providers in accordance with CMS regulations.
  • Ensures that all documentation involved in the home health referral process is complete and accurate.
  • Communicates with patients/ significant other, health care team and physicians regarding the disposition of all home health referrals.
  • Provides patient/family education and orientation to home health services prior to discharge.
  • Serves as a resource to the health care team regarding home health regulations.
  • Represents the Regional Medical Center in the community and develops and maintains relationships with outside agencies.
  • Attends inservices and other educational programs as well as required department meetings.
  • Prepares a variety of reports and special duties as assigned.
