About The Position

Lakeview Village Home Health is searching for caring, dedicated, and trustworthy professionals to join our team! Lakeview Village in Lenexa, Kansas is a not-for-profit continuing care retirement community that offers Home Health, Sub-Acute Rehabilitation, Long-Term Care, and Assisted Living. As a Community Outreach Liaison, you will meet with referral sources, residents, and their families to promote Lakeview Home Health services under the leadership of the Home Health Administrator/Director of Nursing (DON). You will work throughout the greater Kansas City area to serve and secure potential clients and set up referrals with local physicians, hospitals, and other health communities, ensuing business development by creating a team-oriented, resident-centered setting. Job Type: Full-Time Salaried Job Details: Coordinates referrals with hospitals, physicians, communities, and managed care providers. Collects documentation and runs physicians’ orders to assist in agency growth and data collection. Assists with obtaining admission paperwork for new Home Health referrals. Works closely with Home Health staff and local area resources effectively for the benefit of resident care, while observing established policies and procedures. Completes initial assessments of the residents and their families who are admitted to Home Health. Identifies their educational needs and provides them with resources to fulfill these needs. Works closely with the Home Health Administrator and the Marketing team to coordinate upcoming events, weekly call logs, and sales calls for both sales and education. Utilizes Outlook software to share schedules and events with business developers at Lakeview Village. Works closely with the Home Health Administrator to formulate monthly and quarterly referral goals. Develops and implements a Marketing and Business Development plan for the Home Health department that is consistent with the current market analysis and reflects referral source targets. Coordinates and participates in local marketing events. Maintains professional competency by attending seminars, reading professional journals, and networking with peers Participates in departmental and inter-departmental continuous quality improvement projects. Identifies opportunities for product/service enhancements. Maintains up-to-date information on specifically assigned competitors, including, but not limited to: information about marketing strategies, pricing, census, and product information. Works closely with the Marketing team and the Director of Health Services to develop the census through internal and external sales.

Requirements

  • Must have a reliable method of transportation and a valid State Driver’s License.
  • Must have at least one year of experience in marketing.
  • Must demonstrate a full understanding and effective application of State and Federal Medicare regulations as they apply to Home Health.
  • Must have an intermediate knowledge of mathematics, and a skilled technical knowledge of one or more scientific disciplines that are relevant to the field.
  • Must demonstrate a good working knowledge of Microsoft Office Suite software, and adaptability to new systems.
  • Ability to communicate effectively in English, including reading, speaking, and writing, when performing duties that require this need, and with individuals from diverse backgrounds.
  • Ability to work independently, prioritize tasks appropriately, and exercise sound judgement where procedures cannot be standardized.
  • Ability to work weekends as assigned.
  • Demonstrates both precision and accuracy in the completion of duties each day.
  • Possesses an empathetic approach to the elderly.
  • Able to work in a team setting with residents, relatives, staff, and others.
