About The Position
Home Health Case Manager - RN | Home Care $42.00/hour - $44.00/hour Legacy Lifecare VNA West | Travel Required We are seeking a compassionate and skilled Primary Care Nurse/Registered Nurse (RN) to provide high‑quality primary nursing care to patients in their homes and Assisted Living communities. Requirements
- Active unencumbered MA Registered Nurse (RN) license
- Graduate of an accredited school of nursing
- Minimum 1 year of RN nursing experience
- CPR certification
Nice To Haves
- IV therapy experience a plus
Responsibilities
- Complete initial and ongoing patient assessments and develop individualized plans of care
- Provide skilled nursing services, including preventative and rehabilitative care
- Monitor patient progress and respond promptly to changes in condition
- Educate and support patients and families
- Coordinate care with physicians, caregivers, and home health team members
- Maintain accurate and timely clinical documentation
- Supervise and support home health aides as needed
