About The Position

Per-Diem Home Health Care Registered Nurse Location: BROCKTON AREA Join our compassionate and dedicated team providing professional care to patients in the comfort of their homes. We are looking for a skilled Home Health Care Registered Nurse who is committed to patient-centered care. What we Offer: · Mileage Reimbursement · $5 differential for visits performed on Sunday and Saturday · Supportive team environment Wages · The per visit rate for this position is $27 Responsibilities · Delivers skilled nursing care to patients in their home in accordance with individualized care plans. · Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services. · Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs. · Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards. · Conducts ongoing medical documentation, writing OASIS assessments as appropriate. · Prepares clinical and progress notes, submitting all required paperwork daily.
