About The Position

Elevate Healthcare Services is a trusted, Medicare-certified home health agency committed to delivering skilled nursing, therapy, and personal care services with excellence and compassion. We proudly serve patients across Hampton Roads and are expanding rapidly. We’re looking for a motivated, energetic professional to join our team as an Entry-Level Home Health Sales Representative. Position Summary The Entry-Level Home Health Sales Representative is responsible for promoting Elevate Healthcare Services’ programs—including Skilled Nursing, Therapy, Orthopedic Recovery, Cardiac Care, Stroke Rehab, and Fall Prevention—to referral partners throughout the region. This individual will help increase brand awareness, generate referrals, and build strong partnerships. This opportunity provides mentorship and training from a sales leader with over 20 years experience in the industry.

Requirements

  • High school diploma or equivalent; associate or bachelor’s degree preferred but not required.
  • Strong interpersonal and communication skills.
  • Comfortable speaking with healthcare professionals and community partners.
  • Reliable transportation (mileage reimbursement provided).
  • Positive attitude, coachable, and eager to grow in healthcare sales.
  • Highly organized, dependable, and confident working independently.

Nice To Haves

  • Previous customer service, outreach, or similar experience is a plus.

Responsibilities

  • Build and maintain relationships with physicians, case managers, hospital discharge planners, skilled nursing facilities, and community partners.
  • Educate referral sources on Elevate Healthcare’s services, specialty programs, and patient benefits.
  • Conduct routine outreach visits, presentations, and community engagement events.
  • Track leads, referrals, and activities using CRM tools or agency systems.
  • Coordinate with the clinical team to ensure smooth onboarding and communication about referred patients.
  • Represent Elevate Healthcare Services professionally to increase brand visibility and community trust.
  • Support marketing efforts by distributing brochures, attending health fairs, and participating in networking events.
  • Meet weekly activity goals and contribute to monthly census growth targets.
