About The Position

Provide personal care and supportive services to individuals in the community, in the privacy of their home. This role involves delivering health-related services as outlined in a physician-authorized plan of care, established by a registered nurse or licensed therapist, and adhering to the Aide Matrix set by the NYS Department of Health. The aide will assist with personal hygiene, housekeeping, and other supportive tasks for patients with healthcare needs in their homes.

Requirements

• Completion of an approved program of PCA or HHA certification as designated by NYS Department of Health.
• Valid Driver's License and reliable transportation.
• Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program.
• Must have worked in a certified HHA or PCA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry).

Nice To Haves

• High School diploma or GED preferred.

Responsibilities

• Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.
• Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
• Immediately report changes in client’s condition to supervising professional.
• Ensure the safety of the client, client’s family and/or the employee.
• Maintain HIPAA.
• Complete and submit necessary patient documentation including charting tasks and observations on android device.
• Attend ongoing in-service programs in a timely manner, to maintain certification.
• All other duties as assigned.
