We are strengthening our Santa Clara County team and are seeking a Home Health Aide who wants flexibility, organized support, and a stable home health structure behind them. Requirements
- Active California Home Health Aide certification.
- Valid California driver’s license and reliable transportation.
- Current CPR certification.
- Strong reliability and communication skills.
Nice To Haves
- Home health experience preferred.
Responsibilities
- Provide personal care services as directed in the plan of care.
- Assist patients with bathing, grooming, dressing, and hygiene.
- Support mobility, transfers, and safe ambulation.
- Observe and report changes in patient condition to supervising nurse.
- Maintain accurate visit documentation.
- Promote patient dignity, comfort, and safety at all times.
- Communicate effectively with the care team.
