Scheduled Weekly Hours: 36 Hospice Home Health Aide (CNA) Location: La Crosse, WI FTE: 0.8 (64 hours per two weeks) Shift: 8-hour shifts Weekend/Holiday: On-call every 4th weekend and 1 holiday/year Make a Difference in Hospice Care Emplify Health by Gundersen is seeking a Home Health Aide (CNA) to join our compassionate Hospice team. In this role, you will provide essential care and support to patients and families during end-of-life transitions, ensuring comfort and dignity every step of the way. Hospice services are available within a 50-mile radius of La Crosse, WI, and you’ll work alongside a dedicated multidisciplinary team that includes nurses, social workers, chaplains, bereavement coordinators, and fellow hospice aides. Requirements

  • High School Diploma or equivalent
  • Completion of a Home Health Aide/CNA program meeting state and federal requirements for placement on the WI & MN State Home Health Aide Registry upon hire
  • Minimum of 1 year CNA experience in acute care, nursing home, or home health agency
  • WI Registered Certified Nursing Assistant upon hire and MN Registered Certified Nursing Assistant within 90 days of hire
  • BLS CPR certification from American Heart Association or American Red Cross with hands-on learning

Responsibilities

  • Work 0.8 FTE (64 hours per two weeks)
  • Provide care during 8-hour day shifts
  • Participate in on-call rotation every 4th weekend and 1 holiday per year
  • Deliver compassionate, hands-on care to patients and families during end-of-life transitions
  • Collaborate with a supportive team to ensure holistic care
