About The Position

We specialize in home dialysis therapies, empowering patients to receive high-quality care in the comfort of their own homes. We’re seeking a skilled nursing professional in the South Bend, Indiana area to join our growing team. We are seeking a conscientious, detail-focused Home Dialysis Registered Nurse to serve patients in the South Bend, Indiana, area and surrounding regions. This is a home-based care position - you’ll travel directly from your home to patient homes, providing peritoneal dialysis (PD) support, patient training, and acute dialysis services when needed. This role is ideal for a self-directed nurse who values working independently while adhering rigorously to clinical protocols. You’ll develop meaningful relationships with your patients while upholding the highest standards of documentation accuracy and care quality.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in Indiana (or compact state license valid in Indiana)
  • Minimum 1 year of dialysis nursing experience (peritoneal dialysis experience strongly preferred)
  • Current BLS/CPR certification
  • Valid driver’s license with reliable transportation
  • Willingness to travel to Michigan for team meetings and support as needed
  • Exceptional attention to detail and documentation accuracy
  • Ability to work independently in diverse home environments
  • Proficiency with mobile devices for charting applications

Nice To Haves

  • Peritoneal dialysis training/certification
  • Home health or home dialysis nursing experience
  • Experience with Fresenius or Baxter PD cyclers
  • Michigan RN license (or willingness to obtain)

Responsibilities

  • Deliver home-based dialysis care: Provide peritoneal dialysis (PD) support and monitoring in patients’ homes throughout the South Bend area and Northern Indiana/Southwest Michigan region
  • Train patients for independence: Educate patients and caregivers on PD techniques, equipment operation, and self-management (significant training bonuses for each patient successfully trained)
  • Perform acute treatments: Provide in-home hemodialysis in acute or emergency situations as required
  • Complete scheduled patient visits: Conduct monthly assessments, lab draws, and comprehensive care reviews
  • Maintain meticulous documentation: Use mobile charting and AI-assisted scribing to ensure thorough, accurate records within 24 hours
  • Coordinate with the care team: Collaborate with nephrologists, dietitians, social workers, and nursing leadership for integrated patient care
  • Manage your schedule: Organize efficient travel routes across your territory
  • Provide patient support: Offer telephone triage and troubleshooting for home dialysis patients
  • Uphold compliance standards: Follow all CMS regulations, infection control protocols, and organizational policies
