About The Position

Performing direct support services as established upon completion of the admission assessment. The Home Care Support Floater will provide essential services to clients, ensuring their comfort and well-being while supporting their daily living activities.

Requirements

• High school diploma or equivalent
• 2 years of general housekeeping experience, either residential, corporate, or medical facility-related
• Ability to use mature judgment and be sensitive to older adults, family, and community
• Capability to work with minimal supervision
• Ability to maintain effective working relationships with LifeCare staff, clients, and other community agencies
• Ability to document and maintain confidential records

Nice To Haves

• Community health care service experience with culturally diverse and/or elderly populations

Responsibilities

• Provide light housekeeping services to maintain a clean and safe environment for clients.
• Perform errand services including grocery shopping, picking up medication, and paying bills to support clients' daily needs.
• Deliver and complete required client and agency documents in a timely and accurate manner.
• Utilize mature judgment and sensitivity in interactions with older adults, families, and the community, while working with minimal supervision.
• Maintain effective working relationships with LifeCare staff, clients, and other community agencies to ensure coordinated care.
• Document and maintain confidential records in accordance with agency policies and procedures.
