The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible. Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services. POSITION SUMMARY: The Home Care Supervisor - Non-Clinical is responsible for the management of daily home care operations including service provision, service delivery, quality of care, staffing and personnel support. Supervises staff consisting of Caregivers and Home Care Coordinators. Reports to Nurse Manager.

Requirements

  • Associate's degree in Health-related field
  • 3 years' experience related experience - i.e. admin support, caregiver including working with frail/elderly population
  • 1-2 years as a supervisor or higher-levle staff management experience
  • Life support/CPR certificate upon hire
  • Superior organizational abilities in managing multiple requirements: Assignments, home visits, documentation, supervision, timekeeping/payroll systems, regulatory requirements.
  • Excellent verbal/written communication skills.
  • Ability to work as part of a multidisciplinary team.
  • Ability to use electronic scheduling systems, documentation platforms, mobile devices and reporting tools.
  • Moderate knowledge of Microsoft Office Suite.
  • Demonstrated ability to maintain an upbeat, positive approach to coordination, reinforce positive behaviors and create a highly effective team.
  • Excellent organizational skills with attention to detail and ability to learn quickly.
  • Flexibility, initiative and willingness to participate in all aspects of the organization.

Responsibilities

  • Supervises high volume home care operations to ensure services are scheduled and delivered effectively and aligned with established home care plans.
  • Provides training to caregivers and home care coordinators in carrying out responsibilities. Identifies and supports home care team professional developmental needs. Clinical support needs are requested from the Nurse Educator or Home Care RNs.
  • Acts as a resource, preceptor and mentor to new employees, students and other team members by demonstrating knowledge of specific conditions commonly seen in participant population.
  • Conducts personnel management responsibilities, including hiring, performance evaluations, employee relations and disciplinary processes, payroll approval and staff scheduling.
  • Schedules and facilitates monthly home care meetings.
  • Collaborates with Center Director, Nurse Manager and Homecare RN as issues or concerns arise in the community.
  • Partners with nursing and medical leadership to ensure efficient home care operations.
  • Demonstrates ability to identify and resolve problems in collaboration with home care team, program management and nursing leadership team.
  • Works collaboratively with other supervisors/managers on issues related to quality programming efficiency and cost effectiveness.
  • Ensures adherence and alignment of practices with policies, procedures and regulatory requirements.
  • Adheres to On Lok safety policies and procedures.
  • Supports audits, documentation-tracking and quality improvement initiatives.
  • Comfortably performs home supervisory visits, observing care delivery, assessing environment and identifying risks for caregivers.
  • Actively participates in improving quality metrics for the assigned center to meet organizational goals.
  • Actively participates/leads in activities related to home care workflows and relevant improvement issues.
  • Provides home care representation and perspective in organizational strategy and planning by participating in various committee meetings.
  • Advances the goals and objectives of the organization by developing supportive department-specific performance goals, implementing quality improvement plans, and participating in establishing and monitoring financial targets.
  • Other reasonably related duties as assigned.
