About The Position

Come join our awesome team as a Home Care Scheduler with Senior Community Care of Colorado in MONTROSE! This is an onsite/in-office position. Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization. Pay: $20.24 - $24.60 per hour, based on previous working experience. Schedule: Monday - Friday 8:00am-5:00pm, may require some after hours when needed; occasional weekend if needed The Home Care Scheduler will provide efficient administrative services to the Home Care department, resulting in the delivery of excellent internal customer service. Senior Community Care of Colorado (SCCCO) is a PACE Program (Program of All-Inclusive Care of the Elderly) serving older adults in Montrose and Delta Counties. Our team offers wrap-around care services to meet the unique needs of each of our participants and support them to remain living independently in their homes and communities. Requirements

  • High School Diploma or GED required. C.N.A. license is preferred.
  • Hold a current certificate in Basic Life Support or obtain within thirty (30) days of hire.
  • Minimum two (2) years administrative experience; one-year staffing/scheduling and/or caregiving experience preferred.

Responsibilities

  • Monitor completion of home care tasks and shifts. Report any concerns related to attendance and performance to Home Care Manager and Personal Care Services Supervisor.
  • Oversee the staffing process; develop, maintain and update the Home Care schedule one-month in advance; coordinate referrals to contracted agency when approved by Home Care Manager; Report use of agency staff to Home Care Manager and assist with monitoring utilization of agency staff to meet home care needs.
  • Ensure participant home care schedules are accurate and up to date consistently reflecting care needs outlined in IDT approved plan of care.
  • Develop and maintain a good rapport with Home Care Staff and coordinate schedules per availability provided by Personal Care Services Supervisor.
  • Provide clerical support and assisting in various projects for Home Care.
  • Maintaining medical records including tracking, filing and data entry.
  • Perform routine sites visits as needed. May cover acute visits and backfill when scheduled staff are unavailable.
  • Communicate schedule changes to participants and/or participant family members and representatives.
  • Communicate participant concerns and issues with family members and caregivers to Home Care Manager.
  • First point of contact / Liaison between Personal Care Supervisor and staff, contracted agencies and participants, ensuring all requests and issues are resolved in an appropriate, timely fashion.
