Home Care Coordinator for Loyola MacNeal Hospital Home Care Liaison – Loyola Medicine Home Care Loyola Medicine Home Care is a certified Home Health Care agency affiliated with Loyola Hospital. Basic Job Function Summary The Home Care Liaison is responsible for refining and improving the transition processes across all sites of home and hospice care. This role manages the efficient and effective facilitation of referrals to the post‑acute homecare/hospice agency, educates acute care staff (including physicians) about available homecare/hospice services, and may participate in business development activities related to growth or new program expansion.

Requirements

  • RN- LPN- MSW, OT, PT or SLP
  • Excellent communication and customer service skills.
  • Strong critical thinking abilities.
  • Knowledge of Medicare rules and regulations.
  • Working knowledge of managed care environments.
  • Experience multitasking across multiple computer systems.
  • Current registration or licensure in the state of Illinois preferred.
  • 3–4 years of professional clinical practice in an acute care environment preferred.
  • 1–2 years of direct patient care experience in a home care setting preferred.
  • 1–2 years intake or sales experience preferred.
  • Valid driver’s license and reliable transportation to/from work sites.
  • Ability to consistently demonstrate commitment to the mission, Organizational Code of Ethics, and Compliance Program.
