About The Position

Home Care Coordinator Dignity Care Home Care is looking for a Home Care Coordinator. The manager will lead a home care team at the beginning stages of launching of a new office. We are growing again, and are expanding our continuum of care be an instrumental part of our broader CommuniCare Network, which includes more than 140 LTC facilities. Team members will provide home care services across the greater Indianapolis, IN region. The Home Care Coordinator will work closely with the existing RN Home Care Administrator and maintain thorough documentation for all involved Home Care aides attendance waiver programs. Purpose of Position The Home Care Coordinator oversees direct homecare personnel and ensures the appropriate care coordination among all waiver programs and needed relevant internal parties. The ideal Home Care Coordinator will have at least 2 years of experience with managing Home Care Aides and Waiver Programs through insurance verifications and NOA. A proven track record of leadership and managerial skills will be heavily factored in the final decision making. Requirements

  • Minimum of 2 years of home care waiver experience.
  • Minimum of 1 year experience managing home care aides.
  • Proven track record of success in home care management.
  • Outstanding leadership and managerial skills.
  • Knowledge of state Home Care agency regulations.
  • Demonstrates the ability to implement good organization, communication, human relations skills to a growing team of Home Care aides.
  • Must possess the ability to lead office staff in order to ensure successful patient outcomes while maintaining branch performance.

Responsibilities

  • Be responsible for direct supervision of the Home Care staff: Scheduling
  • Complete required NOA
  • Participate in the recruitment, interviewing, selection, and orientation of team members.
  • Evaluate their performance relative to job goals/requirements
  • Coach staff, recommend in-service education programs, and ensure adherence to internal policies/standards.
  • Manage the assignment of clinical associates.
  • Ensure service quality and participate in care coordination to ensure proper communication between caregivers, patients, referral sources, and payers.
  • Discuss operational issues, update staff on new/changed regulations, and review records/documentation to ensure regulatory and in-house compliance.
  • Oversee the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to company procedure and policies.
  • Coordinate communications between team members/caregivers to ensure the appropriateness of care by implementing a focus on outcome planning, and ensured compliance by reviewing documentation and care coordination activities.
  • Educate any associated staff in the correction of these deficiencies.
