About The Position

Parx is looking for a motivated Home Care Coordinator to join our team! This role is ideal for someone who can confidently manage their own client caseload, take ownership of patient care coordination, and ensure every client receives exceptional service. The Home Care Coordinator will play an important role in improving the quality of life for individuals requiring non-skilled care while allowing them to maintain their independence and dignity in the comfort of their own homes. This position is responsible for overseeing assigned clients, coordinating services, maintaining accurate records, and serving as the main point of contact between clients, families, caregivers, healthcare providers, and agency staff. The ideal candidate is proactive, organized, and able to independently manage patient needs from start to finish while ensuring compliance with Medicare regulations and company standards. This role focuses primarily on private duty/private pay clients, while also supporting other referral sources as needed.

Responsibilities

  • Manage assigned client caseloads independently and ensure high-quality service delivery
  • Handle new patient referrals, intake processes, and service setup
  • Communicate with clients and families, providing updates and ongoing support
  • Coordinate caregiver schedules based on client needs and requested hours
  • Recruit, interview, and place the right caregiver with each patient based on personality, skills, availability, and care needs
  • Ensure strong caregiver-client matches to promote continuity of care and patient satisfaction
  • Enter and maintain accurate patient, billing, and scheduling information in the system
  • Create and update care plans to meet client needs and company standards
  • Ensure services remain compliant with Medicare regulations when applicable
  • Build strong relationships with clients, families, referral sources, and community partners
  • Respond quickly to urgent scheduling or service needs
  • Collaborate with internal teams to ensure smooth operations and client satisfaction
  • Assist with caregiver compliance, onboarding support, and administrative tasks
