About The Position

Home Care Coordinator We provide non-medical services that allow clients to live more independent lives at home - wherever home may be. The Home Care Coordinator reports directly to the franchise owner and is part of a growing team. Synergy Homecare of Tampa Bay is growing quickly, and we understand the importance of providing exceptional care to our clients. We also recognize that caregivers are at the heart of our work. We are looking for the right person who is ready to advance their career while growing our business and adding value to the team. Vital components of this position include active engagement with caregivers and building relationships with our clients. The successful candidate will be an energetic professional with a positive can-do attitude and an effective communicator who is eager to help make a difference. We will help position the candidate to be successful in the role with training and a collaborative environment. This role provides opportunity for advancement and growth.

Requirements

  • Experience in home care or home health agency or relatable experience required
  • A minimum of 2-years of experience in an office is required - prefer those who have worn many hats
  • CPR or BLS certification up to date
  • Level 2 background screening up to date
  • Valid drivers license and Reliable transportation
  • Ability to use office technology applications (CRM and Scheduling) for operational effectiveness and efficiency
  • Ability/Willingness to work an on-call rotation

Nice To Haves

  • Experience with scheduling preferred
  • Experience with CRM (customer relationship management) system preferred

Responsibilities

  • Be a reliable contact for care providers and clients, with active communication and problem-solving
  • Document client and caregiver key points in scheduling software
  • Provide exceptional caregiving to clients and mentor other care providers - Ability to go into the field when needed.
  • Conduct client intake and assessment process
  • Manage client and caregiver schedules and caregiver availability
  • Develop and maintain positive relationships within the healthcare and referral community
  • Take initiative and think outside of the box to find possible solutions and to act on them
  • Be available "on call" after normal business hours
  • Maintain confidentiality of all information pertaining to employees and clients
  • Perform other functions as appropriate and helpful towards team, client, and organization objectives
  • Carryout duties on-site as needed
