FTE:_ 1.0 Shift _Day Schedule: _8-4:30pm M-F Position Summary The Home Care Coordinator Case Manager (HCC) assesses the in-home care needs for PACE participants and coordinates with internal and external resources to ensure appropriate care is delivered. The HCC provides in-home case management to PACE participants and provides oversight to home care aides to ensure that the participants in-home plan of care is effectuated.
Requirements
- Two years of experience in case management or care coordination in healthcare setting or community agency
Nice To Haves
- Advanced degree in health field preferred i.e. MSW
- License in health field preferred i.e. PTA, COTA, OT, PT, LCSW, LPN, RN
Responsibilities
- Assesses in-home care needs by doing an in-person assessment in the participant’s home on admission and routinely thereafter
- Coordinates appropriate care with internal and external care providers (home care aides)
- Provides oversight to the plan of care by observing care and reviewing records
- Coordinates with compliance to ensure that caregivers are following the plan of care
- Assess for change of condition and completes new assessments as needed
- Provides case management for in-home care needs for supplies/equipment and supports changes to plan of care when there is a change in participant status, i.e. acute illness or injury
- Coordinates with the Inter -Disciplinary Team to ensure that the established plan of care is effectuated
