About The Position

The Case Manager provides support and coordination of client services in a home care environment. This role ensures clients receive individualized, high-quality assistance, acting as the primary liaison between clients, caregivers, and internal staff. The Case Manager supports care planning, monitors service delivery, and assists clients in accessing community resources, programs, and services.

Requirements

  • Associate’s or Bachelor’s degree in Business Management, Human Services, Social Work, or related field (or equivalent experience).
  • 1–2 years of experience in case management, care coordination, or client services.
  • Strong organizational, communication, and interpersonal skills.
  • Proficiency in Microsoft Office and electronic documentation systems.
  • Ability to manage multiple priorities and maintain accurate records.

Nice To Haves

  • Familiarity with elderly care, disability services, or chronic condition management.
  • Experience with caregiver scheduling and client-caregiver matching.
  • Knowledge of HIPAA, state home care regulations, and client confidentiality standards.

Responsibilities

  • Client Intake & Assessment Conduct in-home or virtual visits to assess client needs, preferences, and goals.
  • Document factors that may impact service delivery, such as environmental or psychosocial considerations.
  • Assist in developing individualized care plans in collaboration with clients and their support systems.
  • Care Coordination & Service Delivery Match clients with appropriate caregivers based on skills, availability, and compatibility.
  • Monitor service implementation and ensure care plans are followed and updated as needed.
  • Communicate changes in care plans clearly to clients, caregivers, and internal teams.
  • Client Advocacy & Support Maintain regular contact with clients to evaluate satisfaction and address concerns.
  • Support clients in accessing community services, benefits, and resources.
  • Respond promptly to incidents or requests for service adjustments.
  • Documentation & Compliance Maintain accurate and timely records in the agency’s documentation system.
  • Ensure services comply with internal policies, regulations, and best practices.
  • Assist in preparing for audits and quality reviews.
  • Staff Coordination & Collaboration Work with scheduling to ensure adequate caregiver coverage.
  • Participate in team meetings, billing reconciliation, and program updates.
  • Support social service staff in implementing client care plans effectively.
  • On-Call Support Participate in a rotating on-call schedule to address urgent client or staffing needs after hours.
