Provide routine personal care excluding toileting/diapering, assistance with home chores, companionship and comfort to clients. Perform Activities of Daily Living (ADL) and Incidental Activities of Daily Living (IADL). Report to the Personal Care Supervisor. ESSENTIAL FUNCTIONS 1) Follow the individual Care Plan developed by the Personal Assistant Supervisor. Provide continuous companionship to client. 2) Provide assistance to client inside the place of residence: a) Provide stabilization during ambulation (no lifting or transferring permitted). b) Assist with self-care and personal hygiene of face and limbs (e.g. hair, skin, nail care, shaving with electric shaver only). c) Meal preparation and food purchasing (no feeding permitted). d) Provides stabilization when walking or using a walker. No lifting or transferring in/out of wheelchair permitted. e) Perform light housekeeping and laundry in areas of the house primarily used by the client. 3) Monitor home environment for health and safety: a) Check refrigerator regularly for spoiled foods. b) Keep the area of the house used by the client free of tripping hazards. c) Monitor the fire safety of the residence. 4) Communication and training – Document all services provided. Complete Aide records daily and submit weekly. Communicate all changes in client’s physical or mental condition to Supervisor including suspicions of exploitation, abuse or neglect. Complete at least 8 hours of training per calendar year. DECISION MAKING Perform a limited number of routine tasks prescribed by the care plan. Rely on practical experience to accomplish them. Inform supervisor of changes in client’s behavior, medical condition and when there are fire/safety concerns. KNOWLEDGE Working knowledge of: - Patient care and home management. - HIPAA regulations. - Agency’s policies, procedures and operating guidelines related to Home Care. Stay current in the knowledge of their profession through 8 hours of in-service training every year. ABILITIES - Speaks, reads and writes English fluently. - Works independently in various care locations (private residence, nursing home, long-term care facilities) across the service area of Bay Aging. - Work and communicate effectively with older adults with dementia or Alzheimer’s disease. - Establishes and maintains caring relationships with clients and their families even in difficult circumstances. - Establishes professional relationships with staff in medical and nursing facilities when applicable. - Maintains regular communication with Care Supervisor via regular phone calls and/or visit to Agency’s office. - Is considerate of clients and hold their personal information confidential. - Demonstrates excellent interpersonal skills with tact and diplomacy. - Keeps clients active and engaged in daily activities. WORKING ENVIRONMENT The work is performed in the client’s home and in public areas (shops, professional offices). It requires repeated standing, bending, stooping, pulling, pushing and lifting weights up to 25lbs unassisted. None of the essential functions of the position can be performed virtually away from the assigned places of work. Qualifications Must be 18 years of age. 16 hours of basic training in patient care and home management preferred. Experience and positive employment references may be accepted in lieu of training.
