The HIV Jump Start Specialist will be responsible for working with people living with HIV/AIDS who are disengaged from HIV primary care following their release from recent jail incarceration. The Jump Start Specialist navigates both health and social services to ensure that clients are connected to HIV health care and HIV medical case management. The HIV Jump Start Specialist will conduct patient assessments, which include whole-patient care through case management and social services resources. The HIV Jump Start Specialist works one-on-one with clients to encourage continued commitment and adherence to medical treatment, access to social services, improved communication, and prompt re-engagement to HIV care and the Ryan White Program.
