About The Position

Role and Responsibilities Provide personal care and supportive services to individuals in the community, in the privacy of their home. Essential Functions Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health. Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home. Immediately report changes in client’s condition to supervising professional. Ensure the safety of the client, client’s family and/or the employee. Maintain HIPAA. Complete and submit necessary patient documentation including charting tasks and observations on android device. Attend ongoing in-service programs in a timely manner, to maintain certification. All other duties as assigned. Requirements

  • Valid Driver's License and reliable transportation.
  • Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program.
  • Must have worked in a certified HHA or PCA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry).

Responsibilities
