The hospital provides a comprehensive array of healthcare services both basic and complex to patients from around the corner, across the country and around the world. As the chief clinical training site for the Lewis Katz School of Medicine at Temple University, the hospital provides a dynamic environment for high-quality care, teaching, and cutting edge research. In conjunction with the Heart Failure team assumes responsibility for assessment, coordination, and education of the Heart Failure patient, on the inpatient and outpatient settings. Assists with delivering high quality, efficient medical care to patients in the Heart Failure Clinical Excellence Program within the Temple Heart and Vascular Institute. Works with complex heart failure patients and supports both the physicians and advance practitioners with patient care management. This includes assessments, interventions, referrals, documentation, telephone triage and pre-cert registration processes. Provides ongoing health education and counseling activities; prompt abnormal testing reporting and follow-up; and maintaining follow-up logs for record keeping purposes. May require travel to other TUH Campuses as needed. This position requires the successful completion of the CHFN or CHFN-K - Heart Failure Nursing Certification, within 2 years of date of hire.

Requirements

• Bachelor's Degree in Nursing from an accredited school of nursing.
• PA Registered Nurse License
• Cert Heart Failure Nurse
• Cert Heart Failure Nurse KB
• Multi State Compact RN License
• 3 years experience in cardiology nursing
• 1 year experience with a Heart Failure program

Nice To Haves

• Master's Degree in Nursing from an accredited School of Nursing.
• 1 year experience in critical care nursing.

Responsibilities
