About The Position

Under the general direction of the Manager for the Diabetes and Cardiovascular Disease (CVD) Branch in the Chronic Disease Unit, serves as the team lead and program coordinator of the Heart Disease and Stroke Program (HDSP). Performs highly advanced (senior-level) program planning, development, implementation, and management of HDSP activities. Provides technical assistance to program staff, internal and external partners, and leadership on all topics related to heart disease and stroke. Assigns work to other team members. Supports statewide implementation of cross-cutting approaches to address heart disease and stroke using evidence-based strategies. Leads grant application, implementation, management, reporting and evaluation activities. Develops contract scopes of work, monitors contractor activities and budgets, ensures contract activities are conducted and reviews for quality, and provides technical assistance to contractors and other HDSP staff that work with contractors. Oversees and supports the CVD Learning Collaborative. Develops and reviews educational materials on evidence-based best practices and approaches to prevent and control heart disease, stroke and associated risk factors, including hypertension and high cholesterol, for a variety of audiences. Consults with stakeholders, oversees and monitors contracts, and implements directives and projects designed to promote delivery of evidence-based services and education on a statewide level for people diagnosed with hypertension, high cholesterol, heart disease, or stroke. Assists management and section leadership with completing legislative, division and section level assignments. Works under minimal supervision, with considerable latitude for the use of initiative and independent judgment.

Requirements

  • Knowledge of public health issues and essential public health functions as it relates to chronic disease and related risk factors.
  • Knowledge of program assessment, planning, development, implementation, and evaluation.
  • Knowledge of evidence-based strategies to improve the quality of care for patients with chronic disease.
  • Knowledge of heart disease and stroke and other chronic disease epidemiology and risk factors.
  • Knowledge of evidence-based public health approaches, including the use of program planning frameworks and dissemination of evaluation findings.
  • Knowledge of budget processes stakeholder engagement, grant application, and program coordination.
  • Skill in use of a computer and Microsoft Office software.
  • Skill in gathering and analyzing data for program decisions, program assessment and evaluation, and writing reports.
  • Skill in effective verbal and written communication.
  • Skill in identifying and understanding basic public health data.
  • Skills in developing and delivering presentations to both lay and professional audiences.
  • Skill in contract development, monitoring and oversight.
  • Skill in working with diverse groups and committees comprised of internal and external stakeholders.
  • Skill in partnership building and maintenance.

Responsibilities

  • Serves as Team Lead for the Heart Disease and Stroke Program (HDSP).
  • Leads development of federal grant applications and continuation applications, activities, deliverables, budgets, work plans and progress reports.
  • Serves as subject matter expert on prevention, management, and risk factors for heart disease, stroke, hypertension, high cholesterol for internal and external workgroups, committees, and legislative inquiries or initiatives.
  • Provides guidance, facilitation and technical assistance to the CVD Learning Collaborative.
  • Works with the Diabetes and CVD Branch Manager in preparing reports, bill analyses, cost estimates, and responses related to legislative actions, media and legislative inquiries.
  • Actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
