About The Position

Community Impact Specialist will ensure that Wellstar Health System makes a positive difference with its investment in community-based organizations and strategic/evidenced-based programs as it aligns with Wellstar's Community Health Needs Assessment (CHNA). The Specialist will lead Community Health activities. The Specialist will be responsible for developing and implementing strategies to achieve positive and measurable impact across the Wellstar footprint, as well as, the day to day management of a portfolio of community benefit programs that align with strategic domains. This role is integral to Wellstar's commitment to community health and involves significant collaboration with internal and external stakeholders to improve the well-being of the populations it serves.

Requirements

  • Bachelors Business Administration/Management or Bachelors Public Health or Bachelors Other or Masters Business Administration/Management-Preferred or Masters Public Health-Preferred
  • Minimum 5 years of experience delivering community health and social services, project management or grant management experience in a hospital, community or corporate environment with a Bachelor's Degree Required or Minimum 2 years of experience delivering community health and social services, project management or grant management experience in a hospital, community or corporate environment with a Master's Degree Required
  • Demonstrated ability to work independently and to adapt quickly to new challenges.
  • High level of organization and attention to detail.
  • Strong critical thinking and analysis, planning and problem-solving skills.
  • Familiarity in the knowledge of the principles, concepts, and methodology of program and grant implementation, and demonstrated skill in applying this knowledge in carrying out assignments, operations, or procedures.
  • Ability to manage relationships with a wide variety of executives, Wellstar Health System volunteers, staff and others; Strong relationship skills are necessary, including collaboration, diplomacy and discretion.
  • Ability to take initiative, work independently as well as in a team and manage a wide variety of responsibilities.
  • Proficiency with Microsoft Office and email management.
  • Excellent communications skills - both written and verbal; Ability to make presentations to groups of people.

Responsibilities

  • Community Health Partnership & Program Management Oversee a portfolio of Community Health programs with medium-high complexity.
  • Manage and develop key community partnerships to advance program reach and portfolio.
  • Facilitate presentations to service line leaders and community partners on program impact.
  • Leads and facilitates communication and interaction among program stakeholders.
  • Collaborates with internal departments to advance impact strategies and community health focus areas, ensuring clinical teams are informed, engaged, and aligned with community-facing initiativesparticularly those driven by the Community Health Needs Assessment (CHNA).
  • Develop relationships with stakeholders including residents and attend community meetings and outreach events.
  • Seek supervisory guidance and support for non-routine issues or exceptions while demonstrating initiative and independent problem solving.
  • Partner with the Wellstar Foundation to identify funding opportunities that align with program goals and community needs, draft and submit competitive grant applications in collaboration with grant writer(s), monitor awarded grants for compliance, performance metrics, and reporting requirements, and prepare and submit timely grant reports and impact summaries to funders and internal leadership.
  • Partner with the Office of Diversity, Equity, Inclusion, and Belonging (DEIB), to include, Inclusion Councils, and Business Resource Groups (BRGs) to advance Wellstars mission and foster a culture of belonging; supports the co-creation of internal engagement opportunities that connect team members to community health programs, promote cultural humility, and amplify health equity initiatives across the organization.
  • Community Health Needs Maintain knowledge of Community Health Needs Assessment findings to ensure program relevancy and effectiveness in program selection.
  • Maintain understanding of Social Determinants of Health (SDOH) and strategies necessary to execute the organizations strategic goals in addressing SDOH across the system.
  • Develop and implement evidenced based programing that aligns with strategic domains governed by the Community Health Needs Assessment (CHNA).
  • Represent Wellstar in community meetings, coalitions, and advisory boards to stay abreast of community concerns.
  • Evaluation & Impact Reporting Work collaboratively with program and evaluation administration team to Evaluate all aspects of the community health program portfolio, including individual assessments and overall effectiveness, using both qualitative and quantitative methods.
  • Formulate and track impact measures and key performance indicators (KPIs) aligned with strategic health equity goals and CHNA priorities.
  • Prepare and deliver comprehensive impact reports, presentations, and correspondence for internal leadership, funders, and community stakeholders.
  • Partner with academic institutions and internal/external collaborators to design and implement evaluation frameworks that support evidence-based practice.
  • Contribute to manuscript development and co-author publications that highlight program outcomes, innovations, and lessons learned.
  • Present findings and best practices at local, regional, and national conferences to elevate the visibility of community health initiatives and foster cross-sector learning.
  • Performs other duties as assigned
