As a Healthy Start Homecare Nurse, you will participate in a team-based care model led by the Care Coordinator, providing top-quality maternal and child healthcare to the community. A caregiver in this position works days from 7:30AM – 4:30PM.

Requirements

  • Graduate of an accredited school of Professional Nursing (Diploma, ADN or BSN program)
  • Current licensure as a Registered Nurse (RN) in the State of Florida
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
  • Valid Driver's License (VDL) and proof of auto insurance coverage
  • OB Homecare: two years of Prenatal Nursing experience in an acute care setting and Current Neonatal Resuscitation Program (NRP) certification through the American Academy of Pediatrics (AAP)
  • Other Homecare: one year of clinical experience as a RN in an acute medical-surgical setting

  • One year of home health experience
  • Infusion Therapy experience
  • Bilingual (Spanish and Creole)

Responsibilities

  • Provide direct nursing care and perform accurate and complete starts of care, resumptions, recertifications and routine visits as requested and within specified time frames.
  • Perform medication reconciliation at each visit.
  • Assess patient for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document pain, manage pain and prioritize/document problems based on assessed needs and problems in collaboration with patient, family and health care team.
  • Maintain complete and up-to-date documentation and file at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to physician as appropriate.
