About The Position

Oakwood Creative Care is a nonprofit organization dedicated to innovative, person-centered programs for individuals living with dementia and other cognitive changes. Through our adult day health care “clubs,” family support hubs, and Healthy Living & Resilience Clinics, we cultivate environments where individuals living with cognitive change experience dignity, purpose, meaningful connection, enhanced quality of life, and true joy — while families receive education, guidance, and sustained support that empowers them to thrive alongside those they love. As part of our Healthy Living model, we are expanding specialized dementia-informed clinical services designed to complement and coordinate with each participant’s primary care provider and specialists. The Healthy Living Nurse Practitioner (NP) – Dementia Care Specialist provides supplemental clinical oversight, wellness monitoring, and supplemental clinical oversight, wellness monitoring, and dementia-informed consultation for individuals enrolled in Oakwood. This role does not replace a member's primary care provider. Instead, the NP collaborates closely with PCPs, neurologists, psychiatrists, and other specialists to: Enhance continuity of care Monitor health changes in real time within the program setting Support dementia-related symptom management Reduce preventable hospitalizations and emergency visits Provide caregiver education and practical guidance This position bridges daily program engagement with clinical insight, ensuring that health concerns are recognized early and communicated effectively — strengthening the overall circle of care.

Requirements

  • Active Arizona Nurse Practitioner license (Family, Adult-Gerontology, or Psychiatric-Mental Health NP).
  • Minimum 3 years of experience working with older adults.
  • Experience in dementia care, geriatrics, neurology, or behavioral health.
  • Strong collaborative communication skills and systems thinking mindset.

Responsibilities

  • Conduct baseline and periodic wellness assessments for program participants.
  • Monitor cognitive, behavioral, functional, and physical changes
  • Identify emerging health concerns and coordinate timely communication with the participant’s PCP or specialists.
  • Support non-pharmacologic interventions for behavioral symptoms of dementia
  • Review medications for potential side effects or dementia-related impact (in collaboration with prescribing providers).
  • Assist with care planning conversations aligned with disease progression and quality-of-life goals.
  • Serve as a clinical liaison between Oakwood and external healthcare providers.
  • Facilitate post-hospital discharge follow-up and medication reconciliation support.
  • Participate in interdisciplinary case conferences with program and family support teams.
  • Strengthen systems that reduce fragmentation of care.
  • Provide consultation to families regarding disease progression, safety, behavioral strategies, and wellness approaches.
  • Offer educational workshops through Healthy Living & Resilience Clinics.
  • Support advance care planning conversations in partnership with medical teams.
  • Promote brain health, movement, nutrition, hydration, sleep, and social engagement.
  • Create personalized healthy living and resilience plans with families
  • Contribute to proactive risk-reduction strategies that enhance quality of life and stability.
  • Help refine and strengthen Oakwood’s supplemental Healthy Living clinical model.
  • Develop scalable protocols for observation, documentation, and provider communication.
  • Contribute to outcome tracking and quality improvement initiatives as Oakwood grows.
