Healthy Families is a home visiting program that provides new, first time parents under 24 years old, with information on child development, support, and a link to community resources. Major Job Responsibilities include: Schedule, plan, and complete home visits with families on caseload Provide parenting education to families using curriculum in the following areas: parent child interactions, parent and child safety, parent and child health, child development, anticipatory guidance and discipline, healthy relationships, healthy pregnancy, labor and delivery Support participants in their roles as first time parents Complete screening and assessments for the child and family, including developmental questionnaires and mental health screens Develop a goal plan with each family Maintain documentation related to work with families in electronic client record Complete program required reports within established timelines Connect families to community resources and provide appropriate referrals when needed Organize, facilitate, and participate in group events for HF families Participate in community outreach activities and maintain professional relationships with community partners Collaborate with community agencies working with parents and children including the Department of Children and Families Practice cultural humility when working with families Attend intensive home visiting and topic-based trainings provided by HFA Attend biweekly staff meetings, participate in case reviews and attend other team activities
