About The Position

Valley Medical Group is looking for a Healthcare Resource Coordinator-Medical Assistant to join our Primary Care team and work in our Greenfield Health Center. Under the direction of the Healthcare Navigator, the Healthcare Resource Coordinator is responsible for supporting the coordination of care for Valley Medical Group patients needing services for transitions in care, disease management, to improve and/or maintain health goals and established outcomes. The Healthcare Resource Coordinator works as part of a collaborative multi-disciplinary team, supporting patients connecting with specialists, community resources and service, providing health reminders and outreach, contributing to the established education and coaching plan. Work with patients includes face-to-face visits, home visits if necessary, communication thru multiple methods (phone, text, other technology in use) and addressing social barriers to optimal health. Requirements

  • Experienced Medical Assistant with the knowledge and skills necessary to work in ambulatory healthcare setting supporting a population health program.
  • Current CPR certification (or commitment to obtain within three months of employment)
  • Current MA Driver’s license
  • Excellent communication and interpersonal skills, establishing and maintaining effective working relationships with patients and families, clinical staff, teams and members of the community
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Excellent skills and proficiency with electronic medical records (EMR), care management platforms, and other IT solutions used to support work
  • Travel to other sites including Valley Medical Group Health Centers, hospital facilities, skilled nursing facilities or patient homes as needed

Nice To Haves

  • CMA or RMA certification strongly preferred
  • Proof of competition of an ABHES or CAAHEP accredited Medical Assisting program preferred
  • Experience supporting patient care coordination, case management, and patient navigation, preferred
  • Athena experience a plus

Responsibilities

  • In collaboration with nursing, timely support of patients needing care coordination services and transition in care support from any setting including hospital, skilled nursing facility and Emergency Departments.
  • Utilize a variety of tools including:  Athena EHR, PING, High risk patient lists, quality data, Hospital System secure texting platforms and others.
  • Complete outreach calls to patients following established processes in a timely manner.
  • Assist patients in understanding and adhering to their plan of care including following discharge instructions, making follow up appointments, medication adherence, self-care management, and access to additional care as needed. Provides education, teaches and coaches patients to meet desired outcomes.
  • Identify and connect patients with appropriate healthcare and community resources, such as support groups, financial assistance programs, and transportation services.
  • Order durable medical equipment, services and supplies as needed for patient.
  • Serve as a liaison between patients, healthcare providers, and other relevant stakeholders.
  • Collaborate and communicate routinely with all members of the care team to keep them informed and communication open.  Participate in primary care huddles and supports complex care coordination. Coordinate with external providers for high-risk patients and transitions of care.
  • Maintain accurate and detailed documentation of patient interactions, care plans, and resources provided to patients in EMR and care systems used.
  • Attend hospital and Skilled Nursing Facility (SNF) care management meetings as needed in person and/or virtually.
  • Conduct home visits for patients when appropriate.
  • Support VMG clinical and quality initiatives to support patients obtaining needed care/testing.  Supports high risk screening of patients to proactively identify patients in need of support.
  • Adhere to all departmental and organizational policies and procedures.
  • Attend all required meetings and participate in projects and committees as requested.
  • Ensure compliance with state, federal, and local regulations, guidelines and accreditation agencies.
  • Other responsibilities and duties as assigned.
